I would like to request information from the Immaculate High School database. I have read and understand all of the terms and conditions in the Immaculate High School Alumni Database Access and Use Policy.  I agree to use any information provided only for the approved program as stated below.  Furthermore, I understand that the use of the information for non-approved purposes is strictly prohibited.
